Event Vendor Application

Purpose: To provide information for the Event Committee to determine vendors.

This form is used to gather information about vendors with interest to participate in an AHSHA event. The information will be evaluated by the event committee and selected vendors notified via email.

Championship Games: Sunday March 28, 2010 at the Chandler Rink.

Submittal deadline for the March 2010 Championship and Consolation games is February 28, 2010.

Events will be held on one, or more, days.  We may also use multiple rinks. The Championship (1st and 2nd Place) and Consolation games (3rd and 4th Place) will be held at the end of March, 2010.  There are three divisions: Two (2) Varsity and One (1) Junior Varsity.

Check all situations that apply.  Day 1 and Day 2 indicate two different dates.  Rink 1 and Rink 2 indicate two different locations.
