PURPOSE: The purpose of this form is to record the agreement of the team management to schedule games with the AZ Thunder teams.

ROUTING: This form is automatically routed to the president, vice president, coaching director, and league administrator.

Every month, players and coaches are recognized for outstanding achievement in the month. 

Recommendations are accepted from the coaching staff.  Please provide detailed information about the player as well as contact information for you.
