PURPOSE: The purpose of this form is to record the player and staff position on the team photos for use in the yearbook.

Use this form to submit the list of players as they appear on the team photo and to list players that are not pictured.

List your entire team staff including coaches, managers, and school representatives that you would like to appear in the yearbook. List the title and name, such as Head Coach Bill Smith, Assistant Coach Tanya Walker, Team Managers Tamara Shively, Patti Hesson and Terry Lape, School Sponsor James Clawson.
