FORM PURPOSE: Use this form as an agreement between the team and the league to secure a limited and defined number of practice ice in addition to the sheets included in the League registration fee.  The form records the payment arrangements, whether directly by the team to the rink at the time of the practice, or if the ice will be billed directly to AHSHA and put on the Team account.

DESCRIPTION: Requests for practice ice should be submitted by the team manager or person designated by the team as responsible for committing the team to additional practices.  All extra practice sessions are secured by a team at an additional cost above the amount collected by the player registrations.  The ice costs will be added to the team account and the teams are expected to pay the bill in full each month.

The requests are date and time stamped to allow for fair processing of the requests.  A copy of an approved request will be sent to team management and parents so that all team participants are aware of the additional cost incurred by securing additional ice.

You should know the date, time and rink of the additional ice requested as it will be necessary to complete the request.

This form will be used to communicate the request for practice ice in addition to the number of practices provided by the League. AHSHA will use this form to record the request for the practice ice and will place it on the team account for payment.
