PURPOSE: The purpose of this form is to record the agreement of the team management to schedule games with the AZ Thunder teams.

ROUTING: This form is automatically routed to the president, vice president, coaching director, and league administrator.

The AZ Thunder U16 and U18 teams would like to play the AHSHA teams in exhibition games. These games are voluntary.  Please complete the form information and click SEND.  Your agreement notification will be sent to the AHSHA  scheduler for entry on the leaguewide schedule.  Your exhibition game will not be scheduled in conflict with your regularly scheduled game.  Once scheduled, an email will be sent to all team participants with the date, time and location of the game.
