RSVP Meeting and First Aid

THIS IS A MANDATORY COACHES AND INJURY MANAGEMENT MEETING.

THERE MUST BE AT LEAST ONE PARTICIPANT AT ONE OF THE SESSIONS FROM EVERY TEAM TO RECEIVE A MEDICAL BAG OR PUCK BAG.

PURPOSE: The purpose of this form is to record the reservation at one of the three sessions.

ROUTING: This form is automatically routed to the president, vice president, coaching director, Injury Management Chair, and league administrator.

Each team should have the HEAD COACH attend this seesion.  If the Head Coach is not able to attend, one of the assistant coaches needs to attend.  Team Managers are also encouraged to attend. 

Each team must have someone in attendance to recieve a medical bag.

The Coaching Committee Chair will discuss AHSHA Coaching philosophy and Injury Managment Committee Chair will provide the informational session on player safety and injury response.

Please use this form to reserve your spot at one of the three (3) AHSHA sessions.  This session is very important to your team.

Session length is four (4) hours. 

This form will collect information about the attendee and selection date.

Medical Bag and Puck Bag: Each team will receive a medical bag. This bag it to be on the bench at all times.  The training session will instruct you on how to replace used supplies and how to report an injury.  You will also receive a puck bag and pucks for your team.  Let us know what you need.
