PURPOSE: The purpose of this form is to record the request of the Head coach to move a player from one team to another team.   

Teams who do not have an associated varsity team:  Use this form to request a move for a player from your varsity team  to a lower division team.

Schools with multiple teams (a varsity and a junior varsity) are not required to use this form to assign a player to a lower division team at the end of the preseason.  Moves after the beginning of the regular season will need to be requested by the Head Coach of the uppermost team.  Moves of this type do not need to be reviewed by the Director of Coaching. 

ROUTING: This form is automatically routed to the Director of Coaching and the league administrator.

This form should be completed prior to discussing a move with the player or parent.  The Director of Coaching will contact the team head coach directly to discuss the request and approval is required before a player may be moved. 

Single teams: The player's name will remain on the varsity roster as a sub, and a reminder that the player is to be given the opportunity to be "called up" for games and is able to participate in the varsity team practices and events.

Multiple teams per school: Only players who have been formally "called up" will show as a "sub".
